Center for Academic Success (CAS) Tutoring Referral

Please note: This form must be completed by the student’s instructor for the course being requested for tutoring. Each student is required to meet with the instructor at least one or two times to discuss their struggles with the course before a tutor request can be submitted. After meeting with the student, if the instructor recommends that the student would benefit from tutoring, the instructor then fills out the form below. An email confirmation will be received by the instructor and the student with further instruction.

Please provide the names and email addresses of up to five students whom you would recommend to tutor this course. These students have to either have taken the course and passed with at least an A-, placed out of the course and are at a higher level in the subject, or be a current graduate student in the given discipline. A staff member from CAS will contact these students to encourage them to serve as a tutor.

You and the student will receive an email confirming that a referral for tutoring has been sent to CAS. The student should contact CAS to set up a time to meet with the Program Coordinator to receive additional information about the tutoring program.
